I hereby release and agree to hold Berzerk Productions in harmless from, and waive on behalf of myself, my heirs, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses and compensation for damage or loss to my family and/or property that may be caused by any act, or failure to act of the company, or that may otherwise arise in any way in connection with any services received from Berzerk Productions. I understand that this release discharges Berzerk Productions from any liability or claim that I, my heirs, or any personal representatives may have against the company with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection to, any services received from Berzerk Productions. This liability waiver and release extends to all Berzerk Productions staff together with all owners, partners, and employees.
I agree to the payment of £140 Early bird (enrolment by 12th July 2026) or £150 on 13th July 2026 I understand a full week's notice in writing will be required should my child not wish to attend and that this should be sent directly to Berzerk productions. No refunds on cancellations are available.
The Summer theatre school may be oversubscribed and your child may have to go on a waiting list. Priority in allocation will be determined by: A. Whether your child has received previous drama tuition B. Prompt return of the enrolment form
Berzerk Productions will not be responsible for any expenses, consequential loss, legal liability, loss or damage to any property or persons arising from legal matters, acts and/or defaults outside their control. I have read this form and agree to the conditions as set out above.
